I notice that some score entries have the option to "Order a printed copy". How do I get to enable
this option and what is the procedure / protocol for this?

Generally, this is limited to public domain works.

Perhaps you could recommend printing your scores in booklet mode, in a copy shop? See the paragraph about this in http://imslp.org/wiki/Help:Downloading_ ... _pdf_files

Is the Creative Commons Attribution-ShareAlike 3.0 an implied consent for this practice? I don't see any "Order a printed copy" link with my typesets. How can I give my permission for this reproduction mode?
But therafter you can't say you've "selled" your score because you don't obtain any benefit. You've only made a nice booklet available for a fee.

Neither of these places should be including anything that is Creative Commons - even though they'd technically be free to do so with the plain-vanilla cc-by licenses.
