I attempted to add a second .mp4 file to an existing item (on #298214). The submission duplicated the file ID PMLP483233. Can this be fixed and how do I avoid errors like this in the future.

So you need to make sure every upload has its own unique file name before submitting them on a workpage to avoid duplicates.
