corrected file not showing up

Back on 26 June I uploaded a corrected version of this file:

As of yet it has not appeared: the old file remains. The system seemed to go through all the processes - successful upload, new file blocked and then cleared. Any ideas as to what went wrong? Does the replacement file need to have the same exact name as the old file? (I did change it to distinguish the old from the new.) I did post on the discussion page back on 14 July and it was suggested that the cache had not yet cleared:
